An insulin blunder has led to a Pitsea nurse being struck off after she was found guilty of misconduct.

Margaret Silver, 54, who worked at Orsett Hospital in Thurrock was dismissed from her job with the South Essex Mental Health and Community Care NHS Trust in May last year following a number of potentially dangerous mistakes.

The professional conduct committee of the UK Central Council for Nursing, Midwifery and Health Visiting heard how Silver had not appeared unduly concerned after she mistakenly injected a patient with insulin.

When staff nurse Ann Russell confronted Silver about the mistake, Silver replied "Oh, I haven't have I? I'm sorry".

Silver was suspended after the incident.

The panel found Silver guilty of two charges of misconduct over the insulin error, as well as wrongly recording the administration of fluoxetine to a patient.

However she was cleared of writing an intimidating and unprofessional letter to Mrs Russell.

Committee chairman Alison Norman said: "The responsibility of this panel is to ensure public protection. We have had no option but to remove her name from the register with immediate effect."
