Hospital chiefs have admitted a catalogue of blunders which meant a woman's lung cancer went undiagnosed for more than a year.

Medical tests revealed Sally Watson, of Colchester, was suffering from the killer disease at least 18 months ago.
But the vital X-ray evidence vanished, leading to a shocking delay diagnosing the illness, the Gazette can reveal.
Mrs Watson, 54, had complained of blackouts and excruciating pain.
But doctors unaware of the missing X-rays wrongly believed she had epilepsy.
The Gazette highlighted Mrs Watsons plight in September.
Essex Rivers NHS Trust now admits serious errors were made and bosses have ordered a review in a bid to make sure it does not happen again.
